CMT Practice Questions
This is a reminder to parents and guardians of 8th grade students attending the Washington, DC trip this year. The total cost of this year's trip is $556.00 and payments are accepted in installments up until the final payment due date of March 1, 2012. Any payment received after that date must be in cash. You may access your child's balance on their portal. Any questions or concerns, please contact the school at 860-945-4830. Thank you.
Please note that fees have been posted for lost books and/or other materials. Please check your child's portal for a complete listing of outstanding fees for these items.
In order to be eligible to try out for any sport at Swift, students need to have the following forms on file at the school: Sports Registration Form accompanied by the Pay to Participate fee, Student Participation and Parental Approval Form and a Connecticut Pre-Participation Evaluation Form. The Student Participation and Parental Approval Form needs to be completed by both parent/guardian and student; the front page of the Connecticut Pre-Participation Evaluation Form is completed by the parent/guardian; the back page is completed by the physician. These three forms and the Pay to Participate fee need to be on file in the school office prior to the designated date before tryouts. The Pay to Participate fee will be returned to you if your student does not make the team.
If you are unsure if your child has a current physical on file, please call Ann Terzigni, School Nurse at (860) 945-4835 before the deadline.
The state-mandated vision/scoliosis screenings for grades 6, 7 and 8 will be done over the next few months. If you have any questions or concerns, please contact Mrs. Terzigni at (860) 945-4835.

For your convenience, a Fees Report is now available on your child's portal which will inform you of any outstanding balances in your child's account regarding lost/damaged school property.
Reminder:
Student grades are always accessible through our Student/Parent Portal. Choose "Student/Parent Portal" from the navigation bar to access Portal instructions and "Infinite Campus" login page. The Portal is your connection to your child's daily progress at Swift Middle School.
