PARENT/TEACHER CONFERENCE SCHEDULER
The Butter Braid fundraiser pick up is scheduled for Thursday, December 5, 2013 between 2:30 p.m. and 6:30 p.m. in the Swift main lobby.
Our eighth grade students will be traveling to Washington, DC for an exciting educational field trip on Tuesday, April 15, Wednesday, April 16, and Thursday, April 17, 2014. During this time, we are scheduled to visit many historical sites. The total cost of this educational trip is $625.00 The final payment is due no later than Monday, February 3, 2014. Please make every effort to make regular installment payments -- due December 3rd, January 3rd and final payment February 3rd. Please make checks payable to Swift Middle School (with your child's name and homeroom number in the memo section).

Please be aware that there are two forms used for medication administration authorization. One is to be used for food allergy medications, such as an Epi-pen or Benadryl; the other form is for any prescription or over-the-counter medication that will be given during the school day. If you shop at Stop & Shop and have not registered your card this year to support a school, please consider registering so that 1% of what you spend goes to Swift Middle School or other Watertown schools (you can register for up to 3 schools). Registering is simple.
Go to the Stop & Shop webpage.
Click on "Join the Program">Register Your Card.
Follow the directions on the next screen.

Enroll in Target's "Take Charge of Education" program and earn 1% of your REDcard purchases for the eligible K-12 school of your choice.
Target Credit Card and Target Visa Credit Card account holders can sign up for the "Take Charge of Education" program by calling 1-800-316-6142 or by visiting Target.com/tcoe or an in-store kiosk. The primary cardholder of a Target Debit Card can sign up for the "Take Charge of Education" program by calling 1-800-316-6142.
Breakfast is available to students daily for a cost of $2.00 for paid students, $ .30 for students who qualify for reduced meals and free for students who qualify for free meals.

In order to be eligible to try out for any sport at Swift, students need to have the following forms on file at the school: Student Participation and Parental Approval Form and a Connecticut Pre-Participation Evaluation Form. The Student Participation and Parental Approval Form needs to be completed by both parent/guardian and student; the front page of the Connecticut Pre-Participation Evaluation Form is completed by the parent/guardian; the back page is completed by the physician. These two forms need to be on file in the school office prior to tryouts. The Pay to Participate fee will be collected if your student makes the team.

For your convenience, a Fees Report is now available on your child's portal which will inform you of any outstanding balances in your child's account regarding lost/damaged school property.
Student grades are always accessible through our Student/Parent Portal. Choose "Student/Parent Portal" from the navigation bar to access Portal instructions and "Infinite Campus" login page. The Portal is your connection to your child's daily progress at Swift Middle School.
